fiel, adjective:
loyal, faithful; accurate

As with faithful in English, fiel covers remaining true to someone, either sexually, or as a friend, servant, and so forth.

Athletic se mantiene fiel a su tradición.
Athletic remains faithful to its tradition, Athletic upholds its tradition.

El matrimonio significia, por encima de todo, el compromiso de ser fiel a la pareja.
Marriage means, above all, the commitment to be faithful to your partner.

If you’re talking about friends, followers, and so forth, fiel means loyal.

su íntimo y fiel amigo Jaime
his close and loyal friend Jaime

para delirio de sus más fieles seguidores
to the delight of his most loyal followers or fans

Finally, you sometimes translate fiel as accurate:

un retrato fiel de la realidad española
an accurate or a faithful picture of how things are in Spain

es el fiel reflejo de la situación económica
an accurate reflection of the economic situation
