El cuello is one of those Spanish words which cover two English concepts: neck and collar.

Tenía un pañuelo anudado al cuello.
He had a neckerchief tied around his neck.

It can also translate as throat:

Le cortaron el cuello.
They slit his throat.

The other meaning is collar:

Llevaba una camisa a rayas, sin cuello.
He was wearing a striped, collarless shirt.
